SUBMISSION OF APPLICATION
Application should be submitted on the prescribed form and must be accompanied by the following:
(A) Two recent passport size photographs in addition to photograph pasted on Admission Application Form.

(B) Two attested copies of Matriculation/Secondary School Certificate or its equivalent showing date of birth.

(C) An affidavit from the father/guardian (Appendix-B).

(D) Signed declaration by the candidate and the parent / guardian [Appendix-B (i) and B (ii)].

(E) Certificate of residence, if applicable.

(F) Attested copies of the following certificates:-

   (i) Detailed marks certificate of Pre-Medical / 10+2 or any equivalent qualifying examination.

    (ii) Character certificate from the Principal of school/college last attended.

Please bring all the original certificates on the date of counselling for verification by the Admission Committee and for submission to M.D. University, Rohtak.

NOTE:
1. Incomplete application will not be considered.

2. Application once submitted will not be returned.

3. Admission Committee shall have the power to reject any application, if proved to be invalid.

MEDICAL FITNESS
The candidate must submit medical fitness certificate from RMP at the time of admission.
GENERAL INSTRUCTIONS
1. The candidates selected provisionally for admission must deposit the fees and original certificates on the date of admission / date allowed by the Admission Committee failing which the admission will be cancelled.

2. No extension of date for depositing the fees shall be allowed under any circumstance.

3. Fees once paid on initial or subsequent admission is non-refundable as per rules.

4. Payment of fees may be made by Demand Draft in favour of ‘Dashmesh College of Physiotherapy’ payable at ‘New Delhi’.

5. In case a candidate remains absent consecutively for 7 working days without any valid reason after the commencement of the classes for the course, her/his admission will be cancelled.

6. All admissions shall be provisional and subject to the final approval of the M.D. University, Rohtak after completing the necessary requirements as per rules/ordinances.

7. Any candidate found medically unfit will not be allowed to continue his /her study in the college.

8. Students who are to re-appear in the failed subject(s) will have to pay Rs. 1000/-per subject and the attendance in these classes is compulsory.

COMPULSORY ROTATING INTERNSHIP (6 MONTHS)
i. Every candidate on passing the final B.P.T. Examination, shall undergo 6 months rotatory training as laid down by the University. At the end of training in any department, the Head of the Department shall forward to the Principal of the college a report on his / her attendance, character and the knowledge of the subject after due assessment. In case the Principal of the college is not satisfied, training in the subject shall be repeated. No stipend shall be paid for internship.

ii. All the interees will be governed by the rules and regulations framed by the college.

iii. Internship training for the students who passed their B.P.T. Annual/Supplementary examinations shall generally be started after the declaration of the result by M.D. University Provisional internship before the declaration of the result is not permitted.

iv. The candidates from other recognized colleges desirous to do internship in Guru Gobind Singh Hospital, Budhera are to pay non-refundable fee as prescribed by the management.
